JOB Summary
Responsible for providing nursing care and coordinates the implementation of the plan of care for each patient, and case management to hospice patients and families. Implements nursing process and coordinates the interdisciplinary care plan for patients and families for care delivery.
DUTIES and RESPONSIBILITIES
Manages a caseload of patients and families, which includes assessing, planning, implementing, evaluating and documenting the care provided.
Initial and ongoing assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial and environmental needs, activities of daily living, and cultural and spiritual implications.
Initial assessment of the patient/family and identification of risks and interventions upon admission.
Implements the individualized plan of care and recommends revisions to the plan as necessary including managing discomfort and providing symptom relief and specialized nursing skills related to palliative end-of-life care.
Develops the initial plan of care with IDT input, and implements the individualized plan of care, and makes revisions to the plan as necessary to manage the patients symptoms and specialized nursing skills related to palliative end-of-life care.
Educates patient/family regarding disease processes, self-care techniques, end-of-life care and the processes for dealing with issues of ethical concern.
Communication of findings and changes in patient condition/needs to the attending physician, Medical
Director and IDT members.
Performs care coordination, case management, appropriate referrals and follow-up.
Case manages a patient caseload, which includes assessment, planning, implementing, evaluating and documenting the care provided.
Educates patients/families regarding disease processes, self-care techniques, and end-of-life care.
Facilitates the implementation of another level of hospice care when necessary.
Ongoing evaluation and assessment of the provision of care, and makes recommendations to the IDT on plan of care revisions based on patient/family/caregiver response to services.
Participates in utilization review and performance improvement activities.
Assesses risks associated with grieving.
Communicates between hospice staff and other health care providers.
Supervises and documents hospice aide/homemaker and licensed practical nurse plans of care and documents supervisory visits per policy.
Informs the physician and other personnel of changes in the patient’s needs and outcomes of intervention.
Documents services provided according to organizational policies and procedures.
Participates in on-call coverage.
Participates in regularly scheduled interdisciplinary team meetings to coordinate the patient/family
plan of care, exchange of information, problem solve, and receive staff support and education.
Ensures that the care plan is up to date and current.
Ensures that all care is provided with respect for patient and family rights.
Reports all grievances/complaints made by patients or families to appropriate persons.
Reports all allegations of patient abuse and/or misappropriation of patient property.
Maintains patient confidentiality according to HIPAA regulations.
Documents services provided according to organizational policies and procedures.

Ability to communicate effectively verbally and in writing.

Ability to develop and maintain rapport with patients and families.
Demonstrated ability to work with an interdisciplinary team.
Must operate a car in a safe manner and maintain a valid driver’s license.
Travel and Working Conditions
Travel necessary on a daily basis.
Exposure to disagreeable conditions including odors, infection, illness, or physical contact from disruptive patients in patient homes/living facilities.
Principal contacts include: patients, families, pets, physicians, office staff, field staff and general public.
Physical demands include: lifting 25-50 pounds; walking, standing, stooping, reaching, talking, hearing, seeing, smelling, depth perception, color vision.
Job Requirements
Required education, experience and licensureCurrent license in good standing as a Registered Nurse.
Minimum of 2 years nursing experience; hospice experience preferred.
